ELEMENTARY SCHOOL HOURS
8:40 am — 2:40 pm
REQUIREMENTS FOR ELEMENTARY SCHOOL REGISTRATION
Age Requirements: All Elementary school grades including
Kindergarten is on or before December 1st
Proof of Age: An original birth certificate with a raised seal is
the required proof of birth
Immunizations: Appropriate immunizations are a New York State
mandated requirement for any school entrance
and include the following:
1. 3 doses DPT, DtaP
2. 3 doses OPV
3. 2 doses MMR
4. 3 doses Hepatitis B (born on or after 1985)
5. 1 dose Varicella (chicken pox, born on or after
1/1/98)
6. Physician’s Report for students entering Kindergarten,
1st and 3rd grade
Transportation: A bus transportation form must be filled out and
submitted by the parent to their school district’s
transportation department by April 1. These
forms can be picked up at BCS.
Entrance Exam: A passing score must be achieved on the entrance test
Student Records: A copy of the latest report card must be submitted.
A record release form will be needed upon
acceptance to BCS.
